A somewhat long answer: I use PhotoImpact for almost all my image editing. PI has a blur filter called "flatten uneven area" which has one control for high pass, one for low pass and one for strength. It's basically a Gaussian Blur filter combined with a high pass filter in other words. It's incredibly slow but gives a great deal of control over the blur effect as you can see from the image where the blur is very selective.
