When using the quick reply to post a reply some folks click the wrong button and think their reply goes through when it actually doesn't. It's pretty easy to do...The image below should be self explanatory.

Yep, even after the buttons were moved it still happens to me now and then.

I think a simple change from "post reply" to "reply" or "go advanced" would cure the problem.
